Who Needs PRIME CARE?
We Provide Nursing Services (RNs and LPNs) to People Who:
- Have a need for skilled nursing care, including pediatric care.
- Need skilled nursing care or nursing visits after a hospital or nursing home stay.
- Need assistance with medication management and/or medication pre-pours.
- Require nursing supervision and nursing assessments (both free of charge) for home health aide services.
Our primary goal is to honor our commitment to treat our patients as individuals and to make their experience with us a positive one. Our philosophy stems from the belief that, aside from the psychological benefits and comfort of being cared for in a familiar home environment, it is also significantly less costly than staying in a nursing home.

We Provide Home Health Aide Services (HHAs, PCAs & Live-Ins) to People Who:
Are elderly, chronically ill, or disabled and require assistance with activities of daily living.
Have come home from the hospital or nursing home or rehab facility and require supervision or assistance with activities of daily living, including feeding assist, bathing, toileting, dressing, walking and mobility.
Desire personal care, home making, light housekeeping, and/or companionship that ensures maintaining a safe home environment.
Need assistance with meal planning, grocery shopping, and errands, in addition to cooking/meal preparation.
Require assistance with medication refills, pre-pouring and/or medication reminders.
Need support and assistance with Alzheimer’s and Dementia care.
Have a terminal illness.
Need respite services.
Could avoid or decrease a hospital or nursing home stay with home care.
